Below the descriptions are links to the forms you need to conduct a JD2D event!!!
Suppose you had 12 people attend your event and you divided them up into four teams (3 people on each team). You would need:
4 Survey Record Sheets (one for each team)
While you’re at the door taking the survey, someone is behind you using this form to record the information you gather at the door.
4 Team Reports (one for each time)
The team report is filled out when you are done canvassing. It allows you to synthesize all the data on the Survey Record Sheet into a single form.
4 Survey Questions and Witnessing Methods (one for each team)
Two sheets of paper give you four forms (If printed duplex and folded along the “short edge”).
I laminate these and take the laminated cards with me when I canvass.
8 Prospect Reports (two per team).
Anyone who desires to learn more about your congregation or agrees to a follow-up visit is considered a “prospect.”
If someone desires a follow-up visit, use this form to get the first and last name—with the correct spelling!—along with their cell-phone number.
On the rare occasion you need more than two, I use the backside of the sheet. Of course, you could send out three or more prospect forms per team. That’s up to you.
1 Weekend Report (one per JD2D Event)
This form synthesizes all the data from the Team Reports into one place. If you’d like your JD2D weekend data to be added to this website, you need email a photo or scan of your Weekend Report (BOTH SIDES) to revtimkoch@gmail.com. You can also mail it snail-mail to Pr. 1 Do’s and Don’t’s
You can send this form with every team, but I just read it off before we go out canvassing. That’s up to you.
